When you have completed your player information and selections, click on the Checkout button at the bottom of this
form. You will then be transfered to Google Checkout to complete your registration. Please look over your order
when you are at the Google Checkout page completely and make sure it is accurate.

	Skins: Each team of 4 can participate in the skins game @ $20 per team
	Mulligans: Max 2 per player for each Saturday tournament and 1 per player Friday @ $5 each

Each player will receive a free Callaway golf pouch. If you prefer, you may upgrade to another accessory instead. Upgraded accessory quantity cannot exceed the total number of players registering. (Maximum of 4 upgrades)
